Who we are

My Home Fiber LLC ("MyHomeFiber," "we," "us"), doing business as MyHomeFiber, is an independent sales organization authorized by Fidium Fiber to market Fidium Fiber internet, home phone and related services in the areas Fidium serves. We are not Fidium, Consolidated Communications, or an affiliate of either company, and we do not own or operate the fiber network.

This website (myhomefiber.com and any page or form we host, together the "Site") is an information hub. It describes fiber service in general terms, lets you ask us to check availability at your address, and lets you request a call, text or visit from our local team. Fidium provides the service itself under Fidium's own customer agreements, which you accept when you order.

Agreeing to these Terms

By using the Site, submitting a form, texting us, or asking us to contact you, you agree to these Terms of Use and to our Privacy Policy, Contact Consent disclosure and Text Message Terms. If you do not agree, please do not use the Site.

You must be at least 18 years old and authorized to make decisions about internet service at the address you give us. We may update these Terms from time to time; the effective date at the top tells you when they last changed, and continued use of the Site after a change means you accept it.

Availability, plans and pricing are informational

Everything on the Site about speeds, features, promotions and availability is general information, provided to help you decide whether to talk with us. It is not an offer, a quote, or a guarantee that service is available at your address. In particular:

• Availability is confirmed by Fidium for a specific address. Our team will check for you, but the final answer, the installation date and any construction requirements come from Fidium.
• Plans, pricing, promotions, taxes and fees are set by Fidium and can change. Current pricing and the broadband facts labels are published by Fidium; our team can walk you through what applies to your address.
• Speeds are the plan's maximum. Actual speeds vary with your equipment, wiring, WiFi and network conditions.
• Orders are placed with and accepted by Fidium. If you order through our team, our team submits the order on your behalf using Fidium's systems; if you order online through a Fidium link on the Site, you are dealing directly with Fidium.

If anything on the Site conflicts with what Fidium publishes or tells you, Fidium's information controls.

When you ask us to contact you

When you submit a form, text a keyword, or give us your details in person, you are asking us to reach out. What each consent box means, which channels we may use, and how to stop are explained in full in our Contact Consent disclosure. In short:

• The required box lets us contact you about your specific request by phone, text and email.
• The optional marketing box lets us also send marketing calls, texts and emails, including calls or texts placed with automated technology or an AI-assisted voice. It is never a condition of buying anything.
• You can withdraw either consent at any time — reply STOP, click unsubscribe, call, email, or use Your Privacy Choices.

You agree to give us accurate information and to only give us contact details that are yours or that you are authorized to provide.

Callbacks, follow-ups and visits

Our team works by phone, by text and email, and in person. Beyond responding to new requests, we may:

• Call back a scheduled call that did not connect — if we could not reach you at the time you chose, we will try again at a reasonable time, unless you tell us to stop.
• Follow up after a door-to-door conversation ("go-backs") — a return visit or call you asked for, or a follow-up after a missed appointment, within a reasonable window.
• Reach out to former and prospective customers whose Fidium order was started but not completed, whose service was cancelled, or with whom we have an existing business relationship, where the law permits, using channels that match the consent we hold.

Every contact identifies MyHomeFiber as an independent authorized Fidium Fiber partner, respects calling hours and the National Do Not Call Registry and applicable state rules, and stops when you ask. Details are in the Contact Consent disclosure.

Using the Site

You may use the Site for personal, non-commercial purposes. You agree not to submit false or misleading information, impersonate anyone, interfere with the Site or its forms, scrape or harvest data, probe or test our systems, introduce malicious code, or use the Site for any unlawful purpose. We may restrict or block access to protect the Site and other users.

Intellectual property

The Site's text, layout, graphics and code belong to MyHomeFiber or our licensors and are protected by copyright and other laws. You may view and print pages for your own use; any other reproduction, distribution or modification requires our written permission.

Fidium, the Fidium logo and related marks are trademarks of Fidium and its affiliated companies and are used on the Site under our authorization as a sales partner. Nothing on the Site grants you any right to use Fidium's or our marks.

Links to other sites

The Site links to fidiumfiber.com and may link to other third-party sites. Those sites have their own terms and privacy policies, which we do not control and are not responsible for. A link is not an endorsement.

Disclaimers and limitation of liability

The Site is provided "as is" and "as available." To the fullest extent permitted by law, we disclaim all warranties, express or implied, including merchantability, fitness for a particular purpose and non-infringement, and we do not warrant that the Site will be uninterrupted, error-free or free of harmful components, or that any information on it is complete, current or accurate.

To the fullest extent permitted by law, MyHomeFiber and its owners, employees and contractors will not be liable for any indirect, incidental, special, consequential or punitive damages, or any loss of profits, data or goodwill, arising out of or related to your use of the Site, and our total liability for any claim related to the Site will not exceed one hundred dollars. Some states do not allow certain limitations, so some of these may not apply to you. Nothing in these Terms limits liability that cannot be limited by law.

Fidium is solely responsible for the services it provides. Claims about the service itself — availability, installation, performance, billing — are governed by your agreement with Fidium.

Indemnification

You agree to defend and hold harmless MyHomeFiber and its owners, employees and contractors from claims, losses and expenses (including reasonable attorneys' fees) arising from your misuse of the Site, your violation of these Terms, or information you provide that is inaccurate or that you were not authorized to provide.

Governing law and disputes

These Terms are governed by the laws of the State of Utah, without regard to its conflict-of-law rules, and by applicable federal law. Before starting any formal proceeding, you agree to contact us at info@myhomefiber.com and give us 30 days to resolve the issue informally. Either party may bring an individual claim in small-claims court. Otherwise, any dispute related to the Site or these Terms will be brought in the state or federal courts located in Salt Lake County, Utah, and you consent to their jurisdiction. Nothing in this section affects your rights under the consumer-protection laws of the state where you live.

General

These Terms, together with the policies linked above, are the entire agreement between you and MyHomeFiber about the Site. If any part is found unenforceable, the rest remains in effect. Our failure to enforce a provision is not a waiver. You may not assign these Terms; we may assign them in connection with a sale or reorganization of our business.
